Scheduling:

We will be happy to accept your phone call request for an appointment. All appointments are made by phone, where our staff can personally assist you.

 
 

Insurance:

We are currently participating providers with the following insurance plans. Please contact your insurance company for details regarding your plan. Our office is not able to provide specific information about your policy’s coverage.

 
 

Payment:

Payment is considered due at the time of your visit to Glacier Dermatology. For insured patients, this means co-payments, co-insurance, deductible amounts, and payment for services not covered will be collected at the time of your visit.

 
 

Minors:

It is our policy that patients under the age of 18 must be accompanied by a parent or legal guardian for initial visits. This in the best interest of all parties, so that we may discuss treatment and/or procedure options for the patient.  

 
 

Cancellations, no-shows,
& late arrivals:

We ask that you be respectful of us and and of our other patients by keeping appointments that you schedule. We understand that things come up and your plans may change, but we require 24 hours’ notice if you would like to cancel or reschedule your appointment.
